Position Overview

As an Advanced Practice Provider, you'll serve as the primary medical provider for residents in assisted living communities. You'll deliver comprehensive, patient-centered care while collaborating closely with facility staff, families, and interdisciplinary healthcare teams.

This position offers a refreshing alternative to traditional primary care by allowing providers the time and autonomy to develop individualized treatment plans while improving health outcomes for a medically complex geriatric population.

Key Responsibilities
  • Provide comprehensive primary care to geriatric patients in assisted living communities
  • Diagnose, treat, and manage acute and chronic medical conditions
  • Perform comprehensive physical assessments and wellness visits
  • Develop individualized treatment plans
  • Coordinate care with specialists, families, and facility staff
  • Prescribe medications and monitor treatment effectiveness
  • Maintain accurate and timely documentation
  • Educate patients and caregivers regarding disease management and preventive care
  • Deliver compassionate, relationship-driven healthcare focused on improving quality of life
Qualifications

Required
  • Active Georgia Family Nurse Practitioner (FNP) or Nurse Practitioner (NP) license
  • DEA license (or ability to obtain)
  • Minimum of 3 years of clinical experience
  • Strong clinical assessment and decision-making skills
  • Excellent communication and interpersonal abilities
  • Ability to travel to one of the designated service areas
