QUALIFICATIONS:Physician profile must include the following documentation and details:License: The Contractor's physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. A North Dakota license is preferred but not required.Board Certification: All contractor's physician(s) shall be Board Certified /Board Eligible by the American Board of Urology (ABU)Copies of degrees and accreditationsNational Provider ID (NPI)Drug Enforcement Agency (DEA) CardMalpractice and medical liability insurance certificatesProof the provider has successful completion of an approved residency program in Urology during which 3 years were devoted to Urological surgeryAnnual Office of Inspector General (OIG) StatementCopies of Advance Cardiac Life Support/Basic Life Support (ACLS/BLS) CertificationsOfferors must submit proof of all required licensing/certification at the time offers are due. Quotes that do not contain copies of the actual licensing/ certification (not just written or typed list with license numbers) shall automatically be rendered technically unacceptable and will not be eligible for award.Staff/FacilityLicense: The Contractor's physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. A North Dakota license is preferred but not required.All licenses held by the key personnel working on this contract shall be full and unrestricted licenses. Contractor's physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application will not be considered for the purposes of this contract.Board Certification: All contractor's physician(s) shall be Board Certified /Board Eligible by the American Board of Urology (ABU) and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance.JOB DUTIES AND RESPONSIBILITIES:Direct Patient Care: Estimated 90% of the time involved in direct patient care. Contractor shall be responsible for evaluating and treating patients in a Urological outpatient clinic daily, seeing patients on consultation and coordinating referral of consultants when necessary. Will make in patient rounds for Urological patients. Will perform cystoscopies, vasectomies, scrotal surgeries, ureteroscopies, transurethral surgeries for prostate disease and bladder cancer. Will provide basic general urological care. Robotics certified is preferred but not required. Will be expected to be in hospital local time. 80 hours a pay period, Monday through Friday. There are no call responsibilities or Federal holiday responsibilities. Contractor is expected to be able to perform: Lymphatic Major Procedures, Abdominal Major Procedures, I&D Prostatic Abscess, Prostate Procedures, Urethra Procedures, Ureter Major Procedures, Epididymis and Spermatic Cord Procedures, Bladder Procedures, Kidney Major Procedures, Penis Procedures, Scrotum Procedures, Testes Procedures, Endoscopy Procedures, and should be able to use holmium laser, ESWL, and able to perform fluoroscopy.Scope of Care: Contractor's physician(s) as appropriate and within scope of practice/privileging shall be responsible for providing Urology care, including, but not limited to:Clinic and Surgical Care: Contractor's physician(s) shall provide clinical urology services including surgeries, consultations cystoscopies and other related urology procedures. Contractor's physician(s) shall be present on time for any scheduled clinics/surgeries as documented by physical presence in the clinic or operating room at the scheduled start time.Specialty Exams: The contractor shall provide direct patient care including specialty exams such as compensation and pension, agent orange, etc., as it pertains to urological conditions only.Medications: Contractor's physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients.Discharge education: Contractor's physician(s) shall provide discharge education and follow up instructions that are coordinated with the next care setting for all urology clinical or surgical patients.
