Greater Boston, Massachusetts - Medical Director (Home-Based Primary Care)

- Seeking a board certified Internal Medicine or Family Medicine physician
- Permanent, employed Medical Director opportunity
- Leadership role overseeing home-based primary care providers throughout Massachusetts
- Maintain a reduced patient panel while providing clinical leadership
- Provide mentorship, supervision, and ongoing clinical support to physicians and advanced practice providers
- Serve as the primary clinical resource for diagnostic questions, complex cases, and treatment planning
- Conduct periodic ride-alongs, chart reviews, case audits, and quality assessments
- Lead quality improvement initiatives focused on patient outcomes, documentation quality, safety, and operational excellence
- Monitor provider productivity, documentation timeliness, and clinical performance metrics
- Develop and implement evidence-based workflows, best practices, and quality standards
- Collaborate with physicians, nurse practitioners, physician assistants, behavioral health providers, specialists, and care coordinators
- Participate in strategic planning, clinical program development, and expansion of home-based care services
- Support provider onboarding, education, and professional development
- Provide comprehensive home-based primary care, including:
- Physical examinations
- Medication reconciliation
- Chronic disease management
- Preventive care
- Patient and family education
- Care plan development
- Maintain accurate electronic medical records

Requirements:- MD or DO
- Board Certified in Internal Medicine or Family Medicine
- Massachusetts medical license (or eligible)
- Minimum 5-7 years of clinical experience
- Leadership experience preferred
- Experience in home-based care, primary care, geriatrics, internal medicine, or complex chronic disease management strongly preferred
- Geriatrics or palliative care experience welcomed
- Strong EMR proficiency

Compensation & Benefits:- Total compensation: $250,000-$350,000 (base salary plus performance incentives)
