Must have current nursing license verified through Nursys within 30 days prior to start.
BLS certification required; must be American Heart Association (AHA) approved.
Completion of Ascension Core Competencies including HIPAA, Joint Commission, NPSG, OSHA topics with minimum 80% passing score.
Completion of Ascension EKG Exam with at least 80% passing score, updated every 3 years.
MOAB Moderate Risk De-Escalation Training required.
Completion of Ascension Cerner Test-Out and Training Checklist with signed attestation.
Documentation of immunizations and health requirements: MMR (two vaccines or positive IGG titer), Hepatitis B vaccination series or declination, Varicella immunity or vaccination, Tdap valid within 10 years, Influenza vaccine recommended during flu season (mask required if declined), 2 Step PPD or IGRA testing with annual TB questionnaire.
Fit Test required annually, preferred prior to start.
Statement of good health or completed physical exam prior to first assignment, updated annually.
Completion of Ascension Skills Checklist within one year of start and updated annually for specialty.
Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and others within 90 days of assignment, updated annually.
Employment verification for past seven years, updated annually.
Education verification relevant to clinical work within 90 days of start.
Driver's License or State ID must be current and match address requirements.
