• Availability to provide requested coverage dates required

• Active Illinois Medical License
• Active Illinois DEA
• Active Illinois Controlled Substance Registration (CSR)

• Board Certified or Board Eligible in Internal Medicine or Surgery
• Wound Care & Hyperbaric Medicine training and experience required
• ACLS (AHA)
• BLS (AHA)
• PALS (AHA)

• Board Certified or Board Eligible in Internal Medicine or Surgery - Required
• Wound Care & Hyperbaric Medicine training and experience - Required
• Active Illinois Medical License - Required
• Active Illinois DEA/CSR - Required
• Availability for requested coverage - Required
• Two references per Stratum process - Required
• Clean, easy-to-credential candidates - Highly Preferred

• Outpatient Wound Care & Hyperbaric Medicine
• 100% Adult & Geriatric patient population
• 12-20 outpatient wound care visits per day
• 1-4 Hyperbaric Oxygen (HBO) treatments per day
• 8 exam rooms
• 2 Hyperbaric chambers

• Wound evaluations and treatment planning
• Compression therapy
• Wound VAC management
• Hyperbaric Oxygen (HBO) treatment oversight
• Debridements
• Cast applications
• Advanced tissue applications
• Incision and Drainage (I&D)
• Callous paring
• Chemical cautery
