    Job Details
  • RN- Case Management Discharge Planning role at Massachusetts General Hospital, part of Mass General Brigham system.
  • Position requires candidates to have at least 6 months experience in large organizations or teaching hospitals with 500 beds or more.
    Job Requirements
  • At least 6 months experience in large organizations/teaching hospitals with 500 beds or more.
  • Primary Source Verification of professional license and/or certification required within 30 days of start date; MA Conditional Licenses not accepted.
  • Nursing licenses require verification through NURSYS and BON.
  • Fit Test (N95) required within 365 days of start date and updated annually; no exemptions allowed.
  • Respirator Medical Clearance Attestation required if fit test is required for skill.
  • Documentation of MMR vaccination or proof of immunity required.
  • TB skin test or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if break >6 months.
  • Physician Health Statement within 365 days of start date and updated if >6 month break.
  • Criminal background check within 45 days prior to start date and updated if >6 month break.
  • Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
  • Resume including past 7 years of work experience required for submission.
  • Two references within 4 years of submission, minimum one supervisor or above in skillset.
  • Skills checklist must be completed within 1 year of submission.
  • Confirmation of Assignment/POI Form required for all initial assignments and extensions.
  • Education verification through background check company required.
  • SSN Trace must be run with criminal check.
  • National Sex Offender Check run within 30 days of start and updated annually.
  • Exclusion checks run within 30 days of start and annually.
  • Candidate submission coversheet required for submission to initial assignment and new facility.
    Schedule Information
  • 40 hours per week, Day shift, Monday through Friday.
  • Shift hours: 07:30 - 18:00.
    Additional Details
  • MA Conditional Licenses not accepted.
  • No religious or medical exemptions allowed for N95 Fit Test.
  • Fit Test updates can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment due to limited resources.
  • County residence history required for background check covering last 7 years.
  • International residence or travel exceeding 30 consecutive days within last 7 years must be included in background check.
