    Job Details
  • Occupational Therapist position in an acute hospital setting specializing in 100% Acute Rehab.
  • No floating to other rehab settings.
  • Must have acute care experience.

    Schedule Information
  • Monday to Friday, 08:30 - 17:00 shift.
  • Occasional weekends.
    Additional Details
  • Flu season mask requirement for exemptions/declinations from October 1, 2024 through March 31, 2025.
  • Completion of Ascension Exit Survey post assignment.
  • Offer letter, initial confirmation, and extension required.
  • Forward facing badge photo with white or blue background required.
  • Documentation of two MMR vaccines or positive IGG Titer required; no declinations accepted.
  • Fit Test required for all travelers, preferred prior to start, can be completed onsite.
  • Documentation of Hepatitis B vaccination series completion or signed declination required.
  • Influenza vaccine optional with mask requirement during flu season Oct 1, 2025 - Mar 31, 2026.
  • Positive PPD requires negative chest X-ray and annual TB questionnaire.
  • Statement of good health or completed physical exam prior to first assignment, updated annually.
  • Driver's License/State ID must be current and match address on file.
  • 2 Step PPD or IGRA required with specific timing prior to start.
  • Varicella immunity documentation required; no declinations accepted.
  • Tdap valid within 10 years required; no declinations accepted.
  • Resume/Profile and two professional references required prior to first assignment.
  • Criminal background check including multiple searches required within 30 days of first assignment, updated annually.
  • EPLS check run within 90 days of start, rerun if break in service >90 days.
  • Clinical skills checklist to be completed within one year of start and updated annually.
  • 12 Panel Drug Screen within 30 days prior to start, updated annually.
  • Education verification relevant to clinical work within 90 days of start.
  • Confirmation of assignment documentation required upon initial assignment and updates.
  • Employment verification for past seven years required, updated annually.
