- Provide palliative care to patients with advanced chronic illnesses (e.g., heart failure, COPD, cancer)
- Collaborate with interdisciplinary team: chaplain, social worker, nurse practitioner, and physicians
- Focus on clinical excellence, patient comfort, and quality care
- Schedule: Monday-Friday, 8:00 AM - 5:00 PM
