Cardiology Nurse Practitioner

Job Summary: We are currently seeking motivated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our team and serve multiple nursing home facilities across NYC. The Nurse Practitioner will be part of Essen growing medical professionals. The NP/PA will conduct routine primary care and follow-up visits for patients residing in long-term care and subacute nursing facilities. The role includes physical assessments, chronic disease management, coordination with facility staff, and collaboration with our supervising physicians and care team.
