A well-established community health center is seeking a Family Medicine Physician to join its mission-driven team in Lancaster, California. This is a full-time, permanent opportunity focused on providing comprehensive, patient-centered primary care to a diverse and underserved population.
Position Overview
Key Responsibilities
As a Family Physician, you will:
Provide comprehensive outpatient primary care for patients of all ages
Diagnose and manage acute and chronic medical conditions
Perform preventive care visits, annual physicals, and wellness exams
Manage conditions such as diabetes, hypertension, asthma, and other chronic diseases
Deliver women s health services and minor in-office procedures (as applicable)
Coordinate care with specialists, behavioral health, and support services
Educate patients and families on disease prevention and health maintenance
Maintain accurate and timely EMR documentation
Support quality improvement and community health initiatives
Qualifications
MD or DO from an accredited medical school
Completion of accredited Family Medicine residency
Board Certified or Board Eligible in Family Medicine
Active California medical license (or ability to obtain)
Commitment to community-based, patient-focused care
