Key Responsibilities - Perform comprehensive history & physical exams
- Review medications and care plans
- Complete STAR & HCC documentation
- Document visits same day in EMR
- Prescribe treatments within scope
- Educate patients and families
- Support care coordination and reduce avoidable admissions
Qualifications - Active Hawaii NP License
- Minimum 1+ year Primary Care experience
- Strong communication and clinical skills
