- Days: 7:00 AM 7:00 PM
- Ongoing coverage beginning July 2026
- Must be available for 2 weeks per month (14 shifts)
- Weekend coverage required
- No call responsibilities

License & Requirements:
- Board Certified Required
- Active Idaho License or Active IMLC Required
- DEA Required
- BLS Required
- Must be willing to cover 14 shifts per month
- Clean background required
- COVID vaccination proof required at presentation
- NPDB report (within last 30 days) required at presentation

- Emergency privileges not available
- Ongoing urgent coverage need
- Malpractice must be included in submitted rates
- Facility utilizes a comprehensive credentialing process
- Travel expenses covered within facility guidelines
- Providers should have credentialing documents readily available upon acceptance
