Job Summary:
One of our direct clients is seeking a dedicated Physical Therapist (PT) to join their Home Health team in Kings County, CA/Colorado Springs, CO. The ideal candidate will provide high-quality, patient-centered care, helping individuals restore mobility, improve functional independence, and enhance overall quality of life in a home-based setting.
Key Responsibilities:
- Conduct initial evaluations and ongoing assessments in patients homes
- Develop and implement individualized treatment plans based on physician orders
- Provide skilled therapy services including therapeutic exercises, gait training, balance training, and manual therapy
- Educate patients and caregivers on home exercise programs and safety techniques
- Monitor patient progress and adjust treatment plans as needed
- Complete accurate and timely documentation in compliance with regulatory standards
- Collaborate with physicians, nurses, and interdisciplinary team members
- Participate in case conferences and care coordination
Qualifications:
- Active Physical Therapist (PT) license
- Experience (home health preferred)
- Strong clinical, communication, and organizational skills
- Ability to work independently in a field-based role
Preferred Qualifications:
- Prior home health experience
- Experience with EMR systems (e.g., Homecare Homebase, WellSky)
