Advance Practice Provider needed for a Skilled Nursing Facility

Deliver high-quality, patient-centered care including assessments, exams, and treatment planning

Perform procedures such as injections, immunizations, suturing, and wound care

Educate and counsel patients on treatment plans, wellness, and preventive care

Collaborate with physicians and care teams to support continuity of care

Salary: $95,000 $150,000 + benefits
