We are seeking a compassionate Family Nurse Practitioner or Physician Assistant to join an established outpatient primary care practice serving adults age 65 and older in Charlotte, North Carolina.
This is an excellent opportunity to practice value-based medicine alongside an experienced physician in a collaborative, patient-centered environment focused on preventative care, chronic disease management, and improving long-term health outcomes.

8:00 AM-5:00 PM
100% outpatient
No hospital responsibilities
Minimal call requirements
Adult and geriatric patient population (65+)
Collaborative practice with an experienced Primary Care Physician

Responsibilities
Deliver comprehensive primary care services to senior patients
Diagnose and manage acute and chronic medical conditions
Develop individualized treatment plans focused on preventative care
Collaborate with physicians, care coordinators, and clinical staff
Maintain timely and accurate documentation
Participate in quality improvement initiatives
Educate patients and caregivers regarding health maintenance and disease prevention
Qualifications
Required
Licensed Family Nurse Practitioner or Physician Assistant
Active North Carolina license (or ability to obtain)
DEA certification
One of the following is required:
Fluent Spanish speaking skills, OR
Minimum of two years of experience practicing in a value-based primary care setting
Preferred
Experience caring for adults age 65+
Value-based care experience
Strong communication and relationship-building skills
