Locum Medical Director PACE Program Opportunity Massachusetts
Full-Time Locum Leadership Role Geriatric-Focused Care Trinity Health Network

A healthcare organization in Massachusetts is seeking a Board-Certified Internal Medicine or Family Medicine physician for a locum Medical Director assignment within a PACE (Program of All-Inclusive Care for the Elderly) program. This is a full-time locum leadership role supporting comprehensive, team-based care for frail older adults in a community-based setting.

Position Overview:
  • Locum assignment (full-time coverage)
  • Monday Friday schedule
  • Leadership role within a multidisciplinary PACE program
  • Focus on frail, medically complex elderly population
Key Responsibilities:
  • Provide medical direction for participant care and clinical outcomes
  • Conduct history & physical exams for new and ongoing participants
  • Develop and oversee individualized treatment plans
  • Participate in intake and eligibility evaluations
  • Lead interdisciplinary care team collaboration
  • Oversee Quality Assurance & Performance Improvement (QAPI) activities
  • Educate participants, families, and care teams on chronic disease management
  • Supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
  • Participate in on-call rotation as required
  • Support operational and leadership objectives of the program
Patient Population:
  • Adults aged 55+
  • Frail, nursing-home eligible individuals
  • Community-based participants with chronic medical needs
  • Focus on maintaining independence and quality of life in the community
Ideal Candidate:
  • Strong experience in geriatrics or elderly care preferred
  • Prior supervisory or leadership experience required
  • Comfortable working in a collaborative, interdisciplinary care model
  • Skilled in chronic disease management for complex patients
Requirements:
  • Active Massachusetts medical license
  • Board Certified in Internal Medicine or Family Medicine (Geriatrics preferred)
  • DEA registration required
  • Minimum 1 year experience with frail/elderly population
  • Prior supervisory experience required
  • Current NPDB self-query within 30 days
