Your Impact as Patient Care Manager In this role, you will:
Lead & mentor clinicians to deliver exceptional home health care
Oversee OASIS documentation accuracy & compliance
Drive quality outcomes, audits, and performance improvement initiatives
Ensure alignment with Medicare guidelines & regulatory standards
Collaborate with physicians, patients & families on care plans
Foster a culture of clinical excellence, accountability, and compassion
Support staff growth through coaching, training, and development
What You Bring • Active RN License
•
Home Health experience REQUIRED • Strong OASIS knowledge/experience (highly valued)
• 2+ years of leadership or supervisory experience preferred
• Deep understanding of Medicare & compliance standards
• Passion for leading teams and improving patient outcomes

Pay Details: $10,000.00 to $11,000.00 per year
