Required credentials
- Current Minnesota LPN/LVN license
- BLS (Basic Life Support) certification
- Long-term care experience preferred
Core responsibilities
- Provide direct nursing care and clinical leadership in a long-term care household, modeling professional nursing standards and person-centered/resident-directed care.
- Observe and assess residents' conditions, coordinate with other services to ensure continuity and quality of care.
- Implement and follow clinical policies and procedures to ensure safe, sanitary, and efficient practices (including medication administration, documentation, RAI/charting, safety, infection control, and resident rights).
- Ensure clinical operations comply with federal, state, and local regulations and the Minnesota Nurse Practice Act.
- Participate in orientation, training, competency evaluation, coaching, education, and performance appraisal of staff.
- Collaborate with interdisciplinary team members, physicians, and other medical personnel to develop and implement resident care plans.
- Communicate and enforce household clinical policies and procedures; foster collaboration and problem solving among staff and with residents/families.
- Support efficient use of household resources and contribute to fiscal responsibility.
- Promote a welcoming, home-like environment and provide emotional, psychological, and spiritual support to residents as appropriate.
