Well respected skilled nursing facility (SNF) is seeking a Physical Therapist who is highly motivated and energetic to join the team in Blackwell, OK . Candidates must be willing to support a friendly, positive and professional environment.

Required Qualifications
Physical Therapist, Skilled
Experience: 1 year
PT(Compact Accepted)

Preferred Qualifications
Proper State License
Skilled Nursing/long term care experience - since October 2019 (PDPM experience)
Rehab Optima (Net Health)
