Facility:This well-established medical center campus hosts a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit within a 99-bed department, providing comprehensive post-acute and long-term care services to the surrounding community. The facility offers strong interdisciplinary team support including nursing, rehabilitation services, case management, social work, and dedicated APP coverage, ensuring physicians have the resources needed to deliver exceptional patient care.
Opportunity:Physician - Internal Medicine - Geriatric Medicine
Job Details:- Provide temporary locum tenens physician coverage for the Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit on campus
- Schedule is three days per week, preferably Tuesday, Wednesday, and Thursday, with 10-hour shifts (7:00 AM - 5:00 PM) and no call required
- Hospitalists cover night call; no after-hours responsibilities for this role
- Provider will have a dedicated APP to support patient care responsibilities
- 12-14 patient encounters per shift across SNF and LTC populations
- Review and approve admissions to the SNF, including completing comprehensive history and physical examinations for all new admissions
- Conduct required physician compliance and recertification visits in accordance with CMS and regulatory requirements for both SNF and LTC residents
- Provide ongoing primary medical care for long-term care residents, including management of chronic medical conditions and preventative care
- Develop, review, and update individualized care plans in collaboration with nursing, rehabilitation services, case management, and social work
- Manage acute changes in condition, evaluate new symptoms, and provide treatment to reduce unnecessary ED transfers and hospital readmissions
- Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety
- Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate
- Complete required documentation, physician orders, certifications, and regulatory documentation to maintain compliance with federal and state requirements
- Coordinate transitions of care between hospital, skilled nursing, long-term care, specialty services, and community providers
- Provide routine medical evaluations and chronic disease management for long-term care residents
- Conduct annual wellness and preventive care assessments
- Manage dementia, frailty, and other complex chronic conditions common in the long-term care population
- Provide oversight of palliative and comfort-focused care plans when appropriate
- Collaborate with families and healthcare decision-makers regarding changes in condition, treatment plans, and advance care planning
- Board-certified or board-eligible Internal Medicine or Family Medicine physician required; experience in SNF, LTC, Transitional Care, and post-acute medicine strongly preferred
- Experience caring for geriatric and medically complex patients is strongly preferred
- Active Montana license or IMLC required; ACLS and BLS certifications required
- Estimated credentialing timeframe is 120 days
