- BC Advanced Heart Failure Cardiology, Ventricular Assist Device (VAD) Certified - REQUIRED
- ACLS - REQUIRED

- Provider Must be local - only mileage is reimbursed - REQUIRED
- DEA - REQUIRED
- Active VA license - REQUIRED
