We have reopened an Emergency Medicine Physician locum need for Carle Health Methodist Hospital, Peoria, IL. Please read all the details below and in the attached practice profile, and then call or email if you still have questions.
REQUIREMENTS: Board Certified/Board Eligible in Emergency Medicine. If confirmed, the Health Attestation Form must be completed and submitted to Stratum Med at least one week before the locum's first day of work, with the physical completed within 90 days of start date. The Health Attestation Form is found in the Help section of Trio. Failure to complete and submit the Health Attestation Form at least one week before start date could result in the cancellation of start date and subsequent dates.
LICENSING RESTRICTION: Candidates must have an ALREADY ACTIVE Illinois license, active IL DEA, and CS. Please DO NOT send candidates that do not meet these licensing requirements; they will be rejected immediately.
SCHEDULE: Coverage needed as soon as credentialed. Shifts are 12-hour shifts, both day and night and including weekends (11p-11a, 7a-7p, and 7p-7a.). Locum must be able to attend an onsite orientation on a weekday prior to first assignment (typically set for 7:30a-5p), which would include administrative paperwork and EPIC Training.
TRAVEL: If candidates are within driving distance, round trip mileage and lodging will be the travel parameters approved, regardless of the means the locum chooses to travel. If candidate is out of driving distance range and needs flights, then please indicate that on the Stratum cover sheet.
RATES: Please provide rates in the following format which is acceptable for Carle to approve (all rates should include malpractice insurance):
- Orientation Hourly Rate (weekday prior to first assignment, typically 7:30a-5p)
- Hourly Rate for schedule shift
- OT Rate for patient care after shift
Charting time after hours cannot be billed extra and needs to be part of the base rates provided.
REFERENCES REQUIRED: Stratum Med requires a minimum of two already completed professional references on any presented candidates at the time of presentation. You may use your own company's locum reference form and share that with us, or you can use our Stratum Med written reference form (found in "Help" area of Trio). We realize this may delay the presentation of candidates being received by us, but this additional information will be required to accompany the CV and the Stratum Med Cover Sheet or else they will be rejected immediately as an incomplete presentation to Stratum Med. These references should be completed by the provider's professional colleagues and medical directors of the most recent workplaces who are able to attest to the provider's current skill set, experience, professional behavior, etc.
As always, we prefer
clean and easy-to-credential candidates please. Please do not send candidates with a lot of malpractice cases or those with high payouts. It's difficult to get privileges approved even when the provider has no pending cases, so please refrain from sending candidates with actively pending cases unless we discuss it first.
