This Home Health Physical Therapist Assistant role is based in Carrollton, Georgia and serves patients in Carrollton and nearby communities in west Georgia. With a mix of historic neighborhoods and newer developments, the area offers varied housing options and a supportive local culture that values both healthcare professionals and the patients they serve.In this position, the Home Health PTA works under the direction and supervision of a licensed Physical Therapist to deliver high-quality, individualized care in patients homes. Each day typically includes traveling to multiple home visits, performing treatments aligned with the established plan of care, and documenting services in an EMR. You will provide therapeutic exercises, balance and gait training, transfer training, and functional mobility interventions, as well as assist with home safety recommendations and caregiver education. The role allows for meaningful one-on-one time with patients, helping them restore function, maintain independence, and safely remain in their homes.The caseload generally focuses on adult and geriatric patients with a range of conditions, such as post-surgical recovery, orthopedic injuries, chronic disease management, neurologic conditions, and deconditioning. Patient volume and visit frequency are structured to support safe driving distances within the Carrollton area and to promote effective time management. You will collaborate closely with Physical Therapists, nursing, occupational therapy, speech therapy, and social work as part of a coordinated home health team dedicated to quality outcomes.Typical scheduling includes daytime hours with some flexibility based on patient needs and agency requirements. Shifts are generally structured Monday through Friday, with occasional weekend or holiday assignments as needed for patient coverage. The position involves frequent driving, entering varied home environments, and adapting treatment plans to the space and resources available in each home setting.This role is well-suited for a PTA who enjoys autonomy, values strong professional collaboration, and is motivated by the opportunity to see the direct impact of their therapy on patients day-to-day lives.
