Physical Therapist - Home Health - (PT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2302.00 - $2373.00
This Home Health Physical Therapist opportunity is based in the College Station, Texas area, a dynamic and growing community known for its friendly atmosphere, strong school systems, and vibrant university presence. Typical responsibilities include performing initial and follow-up assessments, developing individualized plans of care, implementing therapeutic interventions, and updating goals in collaboration with the interdisciplinary team. You will address mobility, strength, balance, transfers, gait training, and safety, with a strong emphasis on fall prevention and patient/caregiver education.Your caseload will primarily consist of adult and geriatric patients recovering from orthopedic surgeries, strokes, cardiopulmonary conditions, and other medical or functional changes that limit safe mobility in the home. You will coordinate care closely with nursing and other therapy disciplines to help patients remain safely in their homes and reduce hospital readmissions. Visits are scheduled to allow for meaningful, unhurried patient interactions and thorough documentation.The typical schedule offers daytime hours with some flexibility to accommodate patient needs and personal preferences. Depending on agency needs, you may work full-time with a set weekly schedule, or in a PRN/part-time capacity offering additional flexibility. The role may include participation in an on-call rotation for urgent therapy needs, though most work is planned and scheduled in advance.
