-Hybrid, full time schedule: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to one hour from home
-Must be able to visit patients in their homes up to 4 days/week, 1 day remote
-Occasional travel every other month to Santa Barbara or San Luis Obispo areas for in-home visits. This could be up to 3 days (2 nights).
