Shifts: 5am to 1:30 pm with an unpaid 30-minute lunch

Duties: General Primary Care, Acute Care, DOT Drug Testing

Light Occ Med

Ages 2 years and up (Employees and their dependents)

REQUIREMENTS

Active IN license

Active DEA

BLS certification

EMR - EPIC
