Case Manager IIWisconsin Community Services (WCS) Residential Division - Parsons House Re-entry Center
Position Summary: Provide direct counseling and Case
Management for Correctional Residents. Motivate residents to achieve treatment
goals and supervise Resident activities. Maintain public safety by ensuring
that all offender actions and activities comply with the strict regulations set
by WCS policy, the correctional funding source and all other regulatory
entities. Essential Functions:Maintain assigned
Caseload.Conduct
individual & group counseling.Complete written
work on time, in accordance with regulatory & funding source guidelines, including,
but not limited to: Social History, Treatment Plan, Progress Notes, Discharge
Summary & required Reports.Schedule required
Meetings with Agent of Record (AOR)/Corrections representative.Attend Staff
meetings.Monitor
Resident's progress toward Treatment Plan Goals.Hold Resident
accountable to established guidelines.Refer to
collateral services (where indicated).Attend trainingOther duties as
assigned.Other Duties and Responsibilities:
Fill-in for absent Staff.
Assist in training new Staff.
Minimal Qualifications:
Bachelor's degree in Social Service field.
Minimum of two years' counseling experience.
Clear WI Caregiver Law. Pass Criminal Background Check.
Pre-employment health and drug screening.
Valid Driver's License, access to auto and auto insurance sufficient to meet agency requirements.
Work Relationship and Scope:The Case Worker has primary contact
with Residents and with Staff. The Case Worker
will maintain contact with Corrections Representative. Personal Attributes: Follow agency Code of Conduct; adhere to established policies and procedures of the agency and of all funding sources; conduct self in an ethical manner; maintain professional and respectful relationships with program staff, other WCS staff, clients, and all external persons and agencies involved with service provision; possess sensitivity toward cultural, ethnic and disability issues; demonstrate commitment to agency values and mission.Knowledge, Skills and Abilities:Cognitive therapy knowledge/skills that address the needs of the AODA/ Correctional Offender in individual and group counselingWorking Conditions:Work is performed in private office and group room. The environment is a busy residential setting where there is frequent contact with Residents, co-workers, Probation/Parole Staff, outside professionals & visitors.Physical Demands:Must be able to climb two to three flights of stairs. Ability to perform CPR.First aid, and emergency evacuation of all building occupants in less than three minutes.
