Job description

A well-established outpatient primary care practice in Greensboro, North Carolina is seeking an experienced Advanced Practice Provider to join its care team. This opportunity is ideal for an APP who enjoys building long-term relationships with patients, working in a collaborative team environment, and focusing on quality outcomes rather than rushed, high-volume medicine.

This is a full-time outpatient primary care position serving an adult patient population, with a strong emphasis on preventive care, chronic disease management, and continuity of care. The practice offers a structured, team-based model with clinical support designed to help providers spend meaningful time with patients and deliver high-quality care.

Job Responsibilities

Provide comprehensive outpatient primary care to adult patients
Manage chronic conditions such as diabetes, hypertension, COPD, CHF, and other common primary care needs
Conduct routine wellness visits, follow-ups, medication management, and preventive screenings
Collaborate with physicians, nurses, care managers, behavioral health, and other clinical team members
Support a patient-centered model focused on quality, access, and long-term health outcomes
Maintain accurate and timely documentation in the EMR

Qualifications

Licensed Nurse Practitioner or Physician Assistant in North Carolina, or eligible to obtain licensure
Previous primary care, internal medicine, family medicine, urgent care, or geriatric care experience preferred
Comfortable managing adult and/or senior patient populations
Strong communication skills and ability to work well in a team-based care setting
Commitment to quality care, patient education, and continuity of care

Full-time outpatient schedule
No nights
No weekends
No hospital rounding
No call
