Medix is seeking an Advanced Practice Clinician for a longitudinal, integrated care delivery program that coordinates the clinical care of high-risk senior members within a facility setting. In this role, you will have the ability to achieve work-life balance with no on-call, weekends, or holidays required.Essential Responsibilities:
Conducting comprehensive assessments and effectively managing acute and chronic medical and behavioral conditions.
Ensuring accurate and complete documentation including ICD-10 conditions.
Communicating and collaborating with the interdisciplinary care team.
Conducting advanced illness and advanced care planning conversations.
Traveling locally between 2 to 3 facilities during shifts (Fayetteville/Brunswick NC areas).
