Compensation: $250,000 $270,000 + Performance Incentives
Schedule: Monday Friday No Nights No Weekends

What You'll Do
• Provide comprehensive medical care for patients within skilled nursing, rehabilitation, and post-acute care settings
• Evaluate new admissions, follow-up visits, and chronic care patients
• Develop and manage treatment plans focused on improving outcomes and reducing avoidable hospitalizations
• Collaborate with Nurse Practitioners, Physician Assistants, nursing staff, therapists, and interdisciplinary care teams
• Lead and support advanced practice providers within your assigned facilities
• Complete accurate documentation and billing utilizing EMR systems
• Promote preventative care initiatives and quality improvement programs
Qualifications
• MD or DO Degree
• Board Certified or Board Eligible
• Family Medicine, Internal Medicine, Geriatrics, Hospital Medicine, or related Primary Care specialty preferred
• Active South Carolina Medical License or ability to obtain
• Active DEA License or ability to obtain
• Strong clinical decision-making and leadership skills
• Experience in Primary Care, Geriatrics, Post-Acute Care, Skilled Nursing, Hospital Medicine, Urgent Care, or Internal Medicine preferred
