Job description

Position: Primary Care Physician

Location: Dayton VA Medical Center

Schedule:

VA HOURS OF OPERATION/SCHEDULING

The hospital operates 24 hours a day, 7 days a week, 365 days a year including Federal Holidays. Primary Care: Normal clinic hours . Patients will be provided care by contractors on-site at the facility in a timely manner in accordance with VA Rules and Regulations with clinic wait times and consult completion. Contractor shall notify the COR at least monthly about any obstacles to meet attendance performance measure.

Shall be available to be scheduled to work during hours of 8am to 4:30 pm. As designated by Primary Care department Chief. All contract staff are allowed two 15 minute breaks and a 30 minute lunch. Breaks are not to be combined or to be used to leave or arrive to work early or late.

Minimum Qualifications:

License: Shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property.

Shall be full and unrestricted licenses. Contractor s physician (s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.

Board Certification and Experience: Shall be Board Certified/Board Eligible in Internal Medicine, have experience in providing scope of care as described herein and be currently certified in VHA-approved Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be always kept up to date. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance.

A current CV which identifies the provider s education and professional qualifications commensurate with the position or specialty for which they are being offered to perform.
