Requirements:
  • Must be Board certified in Orthopedic Trauma Surgery or a Truly board eligible - REQUIRED
  • Must have fellowship in Orthopedic Traumatology or Adult Reconstruction/Total Joint Arthroplasty - REQUIRED
  • Have an active license in both Iowa and Illinois - REQUIRED
  • Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - REQUIRED
  • Open to both graduating fellows and experienced orthopedic surgeons
  • Does the provider require visa support - H-1B cap exempt (Note in Additional Submission Details) - REQUIRED
  • Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
