Physician - Primary Care / Value Based Care Interested in joining an Independent, Multi-Specialty Practice that is known for their patient centered approach, focused on value based care? A state of the art Clinic located in Brooklyn, NY, is seeking a Primary Care Physician to join their growing team. This Practice employs a wide range of Specialists (Cardiology, GI, Endo, Podiatry, OB/GYN, Rheumatology, Nephrology, Neurology, etc.).
Experience with value based care (risk adjustment, HEDIS/star measures, etc.) is highly preferred, but training is available.

Schedule: - Monday - Friday, 8-4:30 PM or 8:30-5 PM
- No weekends required
Compensation: $280,000-$310,000/yr, dependent on experience, plus $20k in bonus potential

Physician Responsibilities: - Review patient's history and perform physical examinations
- Evaluate, diagnose and provide appropriate treatment and patient care
- Collaborate with an interdisciplinary team consisting of specialists, nurse practitioners, RN's, MA's, administrative staff, etc.
- Provide value based care services to a panel of members
- Prescribe medications and formulate ongoing treatment and disease management plans
- Oversee clinical staff development and performance
- Help reduce preventable ER visits and hospitalizations
Qualifications: - An active medical license in the state of New York
- Board certified in Internal Medicine, Family Medicine or related specialty
- 1+ year of experience as a Primary Care Physician
- Excellent communication skills
- Interest in Population Health Management
