Job description

  • Coverage Needed:
    • September 2024 - February 2025
    • Sept 7-8, 10, 13, 22, 23, 30
    • Oct 1-6, 14-20, 16-20, 19-20, 24-25, 30-31
    • Shift: 7a-7p
  • Clinical Information:
    • Call Responsibilities: Code Team Call while on shift & Admit Team Call while on shift
    • EMR: EPIC
    • Patient volume: 16
    • Support Staff: Physician Supervisor & Practice Manager
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IN License
    • IN DEA or easily transferrable DEA that can remain in IN for duration of assignment
    • Procedures MUST be able to perform and show recent case logs: Intubations &
      • Central Line Preferred
